A patient portal is a secure online website that give you convenient, 24-hour access to your personal health information from anywhere with an Internet connection. Using a secure username and password, you can view your hospital health information electronically from participating organizations/sites across South West Ontario.

Where does my health information come from?
Patient health information is provided to the MyChart Patient Portal through ClinicalConnect, a web-based portal that provides physicians and clinicians with real-time access to their patients’ electronic medical information from all acute care hospitals, Local Health Integration Networks’ Home & Community Care Services and Regional Cancer Programs in South West Ontario.
What hospitals are sharing health information with MyChart?

If I signed up for a MyChart account outside of southwest Ontario (i.e. Sunnybrook), do I need to enroll for a MyChart account again?
No. To access your records from hospitals within southwestern Ontario, log in to your MyChart account and follow these steps:
Can I only view health information from the hospital where I enrolled for a MyChart account?
No. A MyChart account allows the user to view health information from all participating/data-contributing hospitals within southwestern Ontario.
